Ongoing support for managing chronic health conditions

Living with a chronic or long-term health condition often requires regular monitoring, treatment and support. Our GPs can work with you to develop a personalised plan to help manage your health and improve your quality of life.

We can support patients living with conditions such as diabetes, heart disease, asthma, COPD, arthritis and other ongoing health conditions.

Chronic Disease Management

Your GP can help with:

  • Regular monitoring and health checks
  • Reviewing medications and treatment
  • Setting personalised health goals
  • Lifestyle and preventative health advice
  • Referrals to specialists and allied health professionals
  • Coordinating your care between different healthcare providers
  • Ongoing reviews to monitor your progress

Chronic Condition Management Plans

For eligible patients, your GP may recommend a Chronic Condition Management Plan (CCMP).

A CCMP helps you and your healthcare team work together to manage a chronic condition. It may also provide access to up to five Medicare-rebated Allied Health services per 12-month period, such as physiotherapy, dietetics or podiatry.

Your GP will assess your individual circumstances and discuss whether a plan is appropriate for you.
